Waiver and Medical Release Form

Camper Information:

I, the undersigned, being the parent or legal guardian of the aforementioned child requesting camp admittance to Futskilz Soccer Training and use of any facilities do so entirely at their own risk. Furthermore, the applicant is in good health, and suffers from no illness, disability or condition that requires the taking ofmedication on a regular basis and any such condition is disclosed and accepted by the camp authority. I also understand that there is no reason that the participant can not or should not participate in vigorous training or play. I, the undersigned, hereby expressly agree to be responsible for any medical bills incurred in the
treatment of any illness or accident. In the event of any such accident or injury, I hereby give my full consent to allowing the camp supervisors and staff to procure any medical treatment deemed necessary and advisable on behalf of my child or ward without prior consent. I understand that, as a condition of admittance as a camper, the undersigned, on behalf of all parents and guardians, and on behalf of the applicant, hereby release Heidi Mueller, the camp director, all and every member of the camp staff, and the Futskilz Soccer School from all and any liability from injury or illness, mental or physical, suffered by the camper during or related to the camp/ training.

FUTSKILZ REFUND POLICY:
Classes – Players who are unable to attend the classes for which they are registered will be eligible for a full refund if the player was absent for the entire session. Refunds will not be given due to partial attendance. If two or more classes in a given session are canceled due to weather, make-up dates will be scheduled starting with the second cancellation. NOTE: Make-up dates may be scheduled in a different location than the original class and will be posted on the www.futskilz.com website homepage.

Clinics – Players who are unable to attend the clinic for which they are registered will receive credit towards a future FUTSKILZ program or to purchase FUTSKILZ merchandise. Clinics that have been canceled due to weather will be given make-up dates and announced on the www.futskilz.com website.

Camps – Players who are unable to attend the camp for which they are registered will be eligible for a 50% refund. In the event that three or more camp dates are canceled due to weather, players will be eligible for a 50% refund.

Elite program – Due to the invitation-only nature of FUTSKILZ ELITE Training, no refunds will be given once a player has accepted a slot in the program. If a player discontinues training for reasons other then injury or illness or is delinquent in payment without discussion for more than 30 days, then their slot will be forfeited to a waiting-list recipient. Re-enrollment will require a new registration fee.

Hiatus- Players may hold their FUTSKILZ ELITE training slot on hiatus for $12 per month.
